To obtain a Social Connection score on the Social Determinants of Health Form, information is needed from the Demographic Form and from the 4 questions on Social Connection (listed above). A. On the Demographic Form, if the person selected either the option of Married or Domestic Partnership for the Marital or Partnership Status variable, score = 1 On the Demographic Form, if the person selected Divorced, Never Married, Separated or Widowed for the Marital or Partnership Status variable, score = 0 B. Combine the answers to Questions 1 and 2 above. a. If combined total is 3 or greater, score = 1 b. If combined total is less than 3, score = 0 C. If answer to Question 3 above is 4 or greater, score = 1 If answer to Question 3 above is less than 4, score = 0 D. If the answer to Question 4 above is yes, score = 1 If the answer to Question 4 above is no, score = 0 Total Score: The four scores (from A, B, C, and D) are summed to calculate a total score.
